| Responsibilities: |
Works in the home of older adolescents and their parents to resolve conflicts which have resulted in a PINS application to Family Court. Specific duties include: assists clients in a family treatment relationship approach, provides progress reports and maintains case records to ensure compliance with policies and regulations, co-facilitates parent groups, determines and insures appropriate community services and supports are accessable to families, provides 24/7 emergency response to enrolled families, and additional duties as assigned. |

This Professional Clinician is part of a multidisciplinary mobile treatment team serving Rockland County’s most seriously mentally ill individuals. Provide individual and group clinical mental services; conduct individualized Family Assessments; participate with ACT Team in formulating treatment planning and goal setting; take lead responsibility for integrating family education and support with the tasks of other team members; offer Family Education and Support to the families of ACT clients; assist client in identifying and clarifying rehabilitation and recovery goals, assist in the development of community support systems and networks; maintain mandated case records and documentation as needed; and be available for 24/7 emergency coverage if needed. |
Requirements: |

Specific responsibilities include checking medical insurance, Medicaid eligibility and benefits for clients; informing clients of fees; conducting pre-certification of services; following up and determining reasons of denials; acting as primary program liaison with insurance companies; setting up pay schedules to ensure payment is processed in a timely manner; maintaining data base records; preparing and submitting reports and additional duties as assigned. |

High School Diploma and two years of clerical experience. One-year experience must be from working in a medical office or treatment facility setting. Strong written, verbal, and organizational skills needed. Valid driver's license. |

Responsible for performing duties associated with an independent living program offering individualized and flexible supportive services that are linked to residency and finances for adults recovering from mental illness. Responsibilities include but are not limited to: providing recipient with information about available community services; assisting recipient with tenancy issues; developing and carrying out service plan with recipient; providing monthly visits; maintaining case records for all recipients as required; assisting/supporting recipient securing subsidies; and meeting with treatment providers and families. |
Requirements: |
Bachelors degree in a health or human services field and 1 year related experience or an Associate’s Degree in a health or human services field with three years related experience or five years direct experience with adult mentally ill individuals. Valid driver’s license and willingness to drive agency vehicles and personal vehicle, as needed. |
