Children’s Case Manager, Bi-lingual, Spanish-speaking

35 hours/week with benefits and 4 weeks vacation

Program: Children’s Care Management

Responsibilities: Work with a caseload of approximately 20-25 children with Serious Emotional Disturbance and/or medical needs and their families.  Provide support, linkage to services, identification of strengths and needs and enhancement of child and family functioning.  Engage child and family in care management services; complete assessments and approve applications for services and entitlements; represent MHA and attend meetings in the community as needed; participate in care reviews, staff development programs and attend scheduled staff meetings;  maintain accurate and timely documentation to effectively complete varied assignments within established time frames; consult with team members and other specialists; prepare crisis intervention plan and provide crisis intervention as required; and perform additional related duties as assigned.

Requirements: A Bachelor’s degree in social work, psychology, rehabilitation, nursing, or closely related field and experience in care management or equivalent; or license as a registered nurse with relevant experience. Experience working with a diverse population. Valid driver’s license.
