For Children: Case Management

Children’s Case Management Services is a community support program for children and youth with serious emotional disturbance who reside in the community with their families. Our goal is to provide support to the child and family in order to reduce out of home placements and increase access to community based services.

The role of case management is to ensure that children and youth with SED have access to services and supports in the community, necessary to enhance or maintain their level of functioning and a sense of personal satisfaction in the roles and environments of their choice.

Utilizing a strength-based approach, the Case Manager and family create an Individualized Care Plan, setting goals to meet the child’s needs. Case Managers can assist with goals in the following areas:

mental health
educational / vocational
socialization / recreational
health
legal
personal

Case Management services are offered in two levels of care, Intensive and Supportive Case Management. The level of case management services an individual receives is based on the intensity of the diagnosis, the resulting need for multiple services, and availability. Intensive Case Managers meet 4 times per month with clients, and Supportive Case Managers meet 2 times per month with clients.

Who is Eligible for Case Management Services ?

Families with a child:

Ages 5-18 experiencing a Serious Emotional Disturbance (SED) or behavioral disorder
At risk of being hospitalized, re-hospitalized or placed outside the home
Whose psychiatric symptoms or behavior severely impacts functioning in the home, school or community
Involved in multiple systems, or in need of accessing multiple systems (mental health, special education, family court, substance abuse services, etc.)
Whose clinical treatment or services have not been successful

How Do I Refer to Children’s Case Management Services?

ALL referrals must go through the Single Point of Access and Accountability (SPOA) process.

SPOA centralizes and coordinates all case management services in Rockland County, thereby simplifying the referral, screening and reviewing process. When a child has been identified as appropriate for more intensive services than he or she is currently receiving, a SPOA application should be completed for access to this level of service in Rockland County.

ANYONE can complete and submit a SPOA application on behalf of an eligible child. Eligibility documentation must be provided. This consist of a psychiatric evaluation completed within the last 12 months indicating a mental health diagnosis.
