ACT is the equivalent to a mental health clinic without walls, whose design allows delivery of services to individuals in their homes/communities. Services provided are geared toward satisfaction of daily living responsibilities, helping individuals develop the tools to obtain and maintain housing, employment, relationships, and relief from symptoms. ACT also provides 24/7 emergency response.
Residents of Rockland County who are over the age of eighteen, have a serious and persistent psychiatric disorder, and a treatment history characterized by frequent use of psychiatric hospitals are eligible for ACT. ACT recipients may also be involved with the criminal justice system or have a history of co-occurring chemical addiction. Individuals with court petitions mandating Assisted Outpatient Treatment may also be enrolled in ACT.
All Services and mental health treatment through ACT are provided by a team of mental health professionals which includes a psychiatrist, social workers/therapists, nurses, art therapists, and peer counselors. Services in the areas of vocational rehabilitation, chemical addictions, and family systems and support are available. ACT team staff will work collaboratively with other service providers and family members, where indicated, to promote a seamless and effective network of rehabilitation, recovery, and support.
Referrals must be completed on Rockland County's Adult Single Point of Access Referral Form, available at key mental health sites or by calling:
