Registered Nurse, PT/Housing Outreach

Part-time, up to 9 hrs/week,

Grant funded through 6/30/17 with continued funding anticipated   

Program:  Housing Outreach

Responsibilities:  Conduct physical assessments, determine physical health needs, refer to community physicians for needed medical treatment, and provide a range of direct treatment, support, and rehabilitation services, including pre-pouring of medications, medication reconciliation, and administration of injections. The RN will help remind the individual of upcoming medical appointments and will accompany them to their medical appointments as needed.

Requirements:  The RN must have the requisite educational degree and training to meet the RN credential, and have experience working with adults with severe mental illness.  Sound knowledge of psychiatric rehabilitation and psychotropic medications usage and side effects. Ability to engage clients through the development of a therapeutic relationship; to deal with crisis situations and adapt to new or changing conditions and to work as a team and participate in a group process of goal setting and problem solving. Excellent verbal and written communication skills. Deep understanding of recovery process and principles of person-centered planning. Comfort going into the community, meeting individuals on inpatient units and in residences. Knowledge of community resources and mental health services.  Proficiency with Electronic Health Records preferred. Experience working with a diverse population.  Valid driver’s license and ability to drive program vehicles.
